People that either take their meds or actually go quite mental, not just eccentric but hearing voices/acting irrationally and potentially a danger to themselves or others...there's those that in lucid/normal states are totally aware of their condition and know what'll happen if they don't take care to stick to their regime, and periodically just decide they don't give a shit anymore.
It's like (though maybe not as bad) as a person getting heart surgery and told they need to stop drinking/smoking/eating fatty foods, and continue on.
