Even if you don't know what you want, don't worry about it. You can always add to it or cover it up later.
For my first tattoo I had in my mind this large backpiece, but in the end I ended up getting something small because I wasn't sure if I wanted to commit that much money and pain to something I've never done before. The tattoo didn't really turn out the way I wanted, but that was because my tattoo artist wasn't very good, and since I hadn't done enough homework, I wasn't aware of it at the time. I was just so excited to get a tattoo (also on my birthday btw). I'm actually going to get it covered up with something that I like better when I get some more money together.
When you get your tattoo, make sure you eat a good meal before hand otherwise you may feel sick during it. When they start tattooing it will hurt some (I think it feels like scratching a sunburn), but you actually get used to it after awhile and it's not so bad. I got mine on my abdomen and over part of my pelvic bone. Which going over bone is a strange sensation as I could feel my entire pelvic bone vibrate. Afterwards though, I got this crazy adrenaline rush and just felt amazing! My friends and I went out drinking afterwards to celebrate. :D
