Blood Omen: Legacy of Kain was an awesome game. It's too bad that the sequels never really lived up to the promise of the first one.
I think the most violent game I've ever played was an old PS shooter called Project Overkill. There was no real plot, you just wandered through endless maze-like levels and gunned down wave upon wave of defenders. When you shot them, blood exploded EVERYWHERE, huge puddles on the floor, splatters on the wall, the whole nine yards. It was really funny to start off with, but the game did get boring pretty quickly.
There was also a lot of violent imagery in the original Silent Hill, what with the random puddles of blood everywhere, the piles of innards, crucified bodies, etc.
