thats a hard one ... i'm sure that the people who like the mondo gore films will come up with shit i couldnt bare to watch ...
but for me .. i think the most effective scene that made me cringe because of the gore ... is still the pencil in the ankle in Evil Dead ..
simplistic but very origional and gory in its own small way :)
