NO the hair isn't form Scream. It is from one of the 2 main characters in the film, and it is a more recent film than Scream.

OK I seem to have made this one a bit too hard then.
Well the hair has been nearly guessed so no clues on that one.

Mouth: 80's short story horror film.(A flim of several short stories)
Left eye(as you look at it) and nose: From a classic trilogy.
Right eye: Fourth of series of films that started in black and white.

Hopefully these will help the get the guesses going again.

I could be absolutely wrong but at a quick glance I would say the left eye is "Evil Ash" from Evil Dead 2 and the hair is from Bride of Chucky.
