Visitor Q (http://www.imdb.com/title/tt0290329/) is probably the most "messed up" movie I've ever seen. Got the rec from that thread - You should check it out if you haven't seen it yet!

Hmmm... Depends on what you're looking for - I'm definitely not much of an exploitation fan, so I stay away from those films (just not my taste). What are you looking for in terms of "nasty/extreme"? It can mean so many things in this genre, you know? What are some movies that you've watched that you'd like to see more of?
If you're into torture/exploitation, check out Mum & Dad (http://www.imdb.com/title/tt1129428/). It's the most recent one that I've seen. Again, not a huge fan of the genre, but if that's what you like, then you should check it out.
Dread (http://www.imdb.com/title/tt1331307/) was pretty tortured. Little indie film that made me feel icky.
Couple of "extreme" films that have gotten a lot of buzz around the forum but I haven't checked out yet are Martyrs (http://www.imdb.com/title/tt1029234/) (I think this one is definitely torture/exploitation from what I hear) and Antichrist (http://www.imdb.com/title/tt0870984/) (this one is supposed to be twisted and arty at the same time - It's currently on Netflix Instant and I will probably check it out soon).
I LOOOOOOOOOOVED À l'intérieur (Inside) (http://www.imdb.com/title/tt0856288/) - Pretty extreme, violent slasher/home invasion flick. Great stuff.
Grace (http://www.imdb.com/title/tt1220213/) might be up your ally, too - Pretty twisted - A lot of people liked it around here (though personally I thought that it could push the envelop more).
Takashi Miike is known as being an "extreme" Asian director (he's the one that directed Visitor Q that I think you should watch). He's also done the following, which are definitely disturbing and worth a watch if this is what you're looking for:
Audition (http://www.imdb.com/title/tt0235198/)
Ichi the Killer (http://www.imdb.com/title/tt0296042/)
Gozu (http://www.imdb.com/title/tt0361668/) (people are torn with this one, though - I thought it was brilliant, but a lot of people I talk to HATED it)
SPEAKING of Asian exploitation, this is a favorite of Doc's - A lot of people around here like it (though I wasn't a fan as I mentioned, not my genre):
Terrifying Girls' High School: Lynch Law Classroom (http://www.imdb.com/title/tt0451788/)
Want some gory, ultra-violent Asian splatterpunk? Check out:
Tokyo Gore Police (http://www.imdb.com/title/tt1183732/)
Riki-Oh: The Story of Ricky (http://www.imdb.com/title/tt0102293/)
Ferox and Elvis Christ can probably recommend stuff you'd like - They seem to like exploitation films. Hopefully they'll chime in. :)
Let me know what you think - So many newbies come around here, ask for recs, and then disappear. :(

hi,have got antichrist, itchi ther killer and fancy inside and martrs but i believe martrys is subtitled and find these a bit difficult to view and read at the same time!although saying that i have seen pans labyrinth and enjoyed it so may give it a whirl?would like to see more films like m.s.p,haute tension and possibly a/underground but think the trash can scene in mordum may be too much for me!?any one seen irreversible or aftermath?not much for the hostel films, found a bit boreing and predictable.a couple of my favourite films, although not nasty by todays standards are the burning, madman, evil dead, friday 13th 1 and 2,the thing etc. what are peoples thoughts on these?
