The SAW video game. Set in an old insane asylum and the protagonist of the video game must solve puzzles and rescue people from being killed in Jigsaw's games. Through out the asylum you will be confronted by enemies trying to kill you to obtain a key that is buried inside of you're chest.

Though there are references in Let's Scare Jessica to Death sayingf she just got out of an asylum, there are no scenes in the film showing this.

One of the Nightmare on Elmstreet movies was in an asylum. Can't remember if it was part 3 or 4?

I know the WHOLE movie doesn't take place in a mental institute but The Exorcist III had some pretty screwed up scenes...

Friday the 13th Part V: A New Beginning ('85) -- It's not really an asylum kind of setting, it's more of a home for the mentally disturbed such as teenage Tommy Jarviss, in the previous film he killed Jason Voorhees with multiple machete blows to the head area and was sent to the home because he was emotionally disturbed.
