The scene in Mordum with Maggot (?) and the little girl in the bathtub left me feeling pretty twisted afterwards.

There was a scene in City of God (2002) where the Local drug lord Lil' Z went to exacts revenge on a gang of 9 and 10 year olds and forcing one of his own kid soldiers, to choose a kid from that little gang & kill. Faced with death, one of the kids starts crying and later on.. Lil' Z shoots on the kid's foot!! Truly...this is a scene that I think literally more horrifying and shocking than any gruesome or torture scenes of horror cinema in recent times.

No real spoilers here. The spinning overhead camera scene in Calvaire was a symphony of brutality. The brutality in the beginning of Irreversible was pretty freaking core as well. Nice list, by the way.

The wine bottle face demolishing death in Pan's Labyrinth. Pretty much everything listed so far was to be expected given the types of movies involved. But that scene in Pan's Labyrinth came pretty much out of nowhere.

Not the most shocking, but the most recent one was the end of the Mist. Especially the reaction of the crowd in the theatre. Just a few scenes earlier everyone was cheering the death of Mrs. Carmody, and after the final scene were walking out silent and disturbed. I of course had a big smile on my face.
Loved that ending.

That sounded funnier in my head.

I gotta go seconds on the face smashing scene in Pan's Labyrinth. It looked fake, but still disturbed the hell out of me. That whole movie depressed me for days.

That scene is actually delightful when compared to the face-smashing scene in Irreversible.

Hitman mentioned the end of The Mist but didn't call it the most shocking... Well, I think I will call it my most shocking. I'm rarely caught off guard, but this movie left my jaw dropped and I love it for that!
