Here are some very brutal films, in my opinion.
Wolf Creek
The Decent
Last House On The Left
Haute Tension/Switchblade Romance
Audition
Those are all the ones I can think of at the moment. Also, what makes a brutal horror movie brutal?

Ok, you're new, so i'll inform you. Don't make 42709260956 threads on your first 10 minutes here, it gets annoying. Also, search for threads before you make them, we have a crappy search engine, but it works nonetheless.

From a horror standpoint, cannibal holocaust, dead alive, and last house on the left.
Non horror, passion of the christ
most brutal to watch, spice world, the olsen twins movie

Brutal...hard to watch?
My usuals:
Last House on the Left and Nekromantik
Audition, yes, the last 25 minutes or so are unbelievably painful to watch. Eesh.
Of course the obvious exploities, Cannibal Holocaust, Cannibal Ferox, etc.

Antropophagus - eating a fetus is legendary.
Beyond the Darkness
Maniac(1980)

For truly brutal and (almost) pointlessly sadistic shit:
Flowers of Flesh and Blood
Cannibal Holocaust
Hostel (while not extreme in gore, was very sadistic)
The Saw movies (see Hostel's explanation)

I'd go with Irreversible, and August Undergroud - Mordum.
As far as what makes a movie brutal, I've seen a few theories on this while reading through the various threads on the subject. My own thoughts are that a truly brutal film is one where the victims have done nothing to merit the violence visited upon them.
Most horror films, you can pick out the victims by their actions. Having sex, going exploring unexplained noises alone in the dark, we are programmed to see these kinds of things as triggers for upcoming bed happenings. Strangely, as brutal as this violence often is, it's not regarded as such by the viewer.
However, when you have victims that are chosen completely indiscriminately, who have done nothing to indicate to us what is about to happen, this is where I feel the brutality of a film.
Also, for me, I just can't see the brutality of a film like, for example, Braindead (Dead Alive). Yes, lots and lots of gore, but it's a supernatural type story. For a film to feel brutal to me, it has to be completely believable - no monsters or ghosts, or unkillable slashers a la Jason, just mortal people being violent to other people. Gets me every time, because it could happen.

hence, the passion of the christ is the most brutal. it really does tick all the boxes, and it actually happened, depending on your view point (but please don't open up that can of worms, again).

Brutal? Alien. Nature's brutality to the max. It doesn't need chainsaws or machete wounds, the vicious tongue thing is brutal enough. Nasty, horrific, painful. You can almost feel the pain when it lacerates someones heart out...errrrghhhh

AHAHA! Yea. While Alien doesn't have the shock it did back in the late 70s, I still remember people's first reaction to that classic dinner scene...ya know, the chest bursting scene I loved seeing people watching it for the first time screaming and grabbing their own chests as if something was about to explode through it. :p

I know. But Holocaust is such a common movie around these parts, I was giving the fellow another different choice.
Has anyone seen Salo though? One thing I can gurantee you...you ll need at least a week to get your brains normal again, after you have finished watching.

Yeah, I saw Salo a few weeks ago. I don't think I'd put it up there with the most brutal necessarily, but it was VERY bizarre.
I still don't know what to make of it really. Not a movie I'd go back to in a hurry, certainly, but I'm glad I've seen it. Not to give it away, all I'll say is there was that eating bit and... eurgh!

One's that I haven't seen on this thread yet:
Bloodsucking Freaks (Hostel ripped off this movie in a big way)
Thriller (No, not the Michael Jackson video :p The 1974 Swedish film that Kill Bill's Elle Driver character was ripped from)

Assuming that there is such a thing as psychological brutality, I'm going to go with Jesus Camp.

Why is"Jesus Camp" PG-13?
It don't sound brutal.Should it be rated(R)?

I'm downloading this at the moment. I just know it's going to be one of those where, by the end of the documentary, I'll be ripping the arms off my chair in anger.
Brainwashing. Pure and simple. If it wasn't under the umbrella of 'Christianity' no-one would have any hesitation using the word, and there would be absolute outcry.
Of course, this is before I've seen it. As you can see, I'll give it a fair and impartial view.

Probably the most brutal, leaving the worst feeling afterward, Men Behind the Sun(the cat, the autopsy, the frozen hands[1 and 2]), it's all pretty repellent. I'll never watch it again, not because it's too much to take, but because there's nothing to enjoy there.
Others:
Flowers of Flesh and Blood
Guts of a Virgin(though it's pretty goofy)
Organ
I'm thinking of one more but I can't recall the title. It had Red in the name because when the guy saw a woman wearing red he'd attack them. Somewhere in all that was a 5 minute long rape scene involving a retarded woman. Anyone know this title?
*Nevermind, just remembered its called Red to Kill
