Mate, there are a ton of threads on this (So many it may deserve its own list;))
Mothman or Specs might direct you to Horrorbrains list, which I have no opposition to.
On it, you'll find stuff such as:
August Underground's Mordum
murder-set-pieces
Men Behind the Sun
Irreversible
Cutting Moments (see The Unsung, the Underappreciated & the Bizarre: The Other HDC Top 100 (http://www.horror.com/forum/showthread.php?t=26771))
Genesis/Aftermath
Guinea Pig: Flowers of Flesh & Blood
Yet there are still other depraved pieces of cinema that you can watch, such as:
Slaughtered Vomit Dolls
Begotten (see The Unsung, the Underappreciated & the Bizarre: The Other HDC Top 100 (http://www.horror.com/forum/showthread.php?t=26771))
The Baby of Macon
Salo: or the 120 Days of Sodom
Nekromatic
Butterfly Kiss
The Untold Story
Squirmfest
Enjoy, and hope you get the throw-up-a-little-in-the-mouth action you're looking for!

Schramm. from the director of nekromantik. not really quite as disturbing as it is all around disgusting.

crazy mother fucker hammers 3 nails into his dick in super-close up.
as for the most disturbing movie i have seen, Irreversible....really stuck with me.

Most disturbing instantly brings 2 pictures into the mind...
- the bathroom death in Final Destination
- the climax of Se7en
Overall most disturbing movie? I d say Salo.
Irreversible is close, but not that disturbing a movie IMO. I d say Alky has a point when he mentions Necromantik.

Like many of you, I have seen several disturbing movies. However, there have only been a few that I've found truly upsetting. As for the most vile, terrifying, traumatic, keep-me-up-all-night, sick-to-my-stomach, oh-shit-I-wish-I-never-watched-it film I have ever seen...drum roll please...the winner is Jesus Camp. Seriously. Nothing else even comes close. I don't know if I'm ever going to be able to get that one out of my head.

thank you, i watch a horror movie and i think about when and where that might happen to me, every kill in the movie, every disfigurement, i am just paranoid, i live near the woods in the country, so i think anything like that can happen any time, alright, i think the first horror movie i saw, but i am not sure was "If I die before I wake" anyone heard of that movie, after i saw it i thought someone was breaking into my house and killing us and with house i was scared of being kidnapped, i didnt really even like riding my bike off my property because i thought someone in a car would kidnap me ok
