Dissociative identity disorder (DID) is a psychiatric diagnosis that describes a condition in which a person displays multiple distinct identities or personalities (known as alter egos or alters), each with its own pattern of perceiving and interacting with the environment. In the International Statistical Classification of Diseases and Related Health Problems the name for this diagnosis is multiple personality disorder.
In both systems of terminology, the diagnosis requires that at least two personalities routinely take control of the individual's behavior with an associated memory loss that goes beyond normal forgetfulness; in addition, symptoms cannot be the temporary effects of drug use or a general medical condition. I agree with disliking the ending. I was the exact same way, went back through the movie and triend to make it make sense. There are some parts that it makes a lot more sense after seeing the ending, but also makes other parts seem pointless. (Like you said, the truck.)
That said, the scene with the killer in the truck at the beginning made me extremely uncomfortable. That set a tone for the rest of the movie, so I think it did it's job not as a plot device, but creating atmosphere.

The ending was bloody awful. The movie should have just been a straight-up slasher. No need to analyze it beyond that.

Because he wants people to agree with him...feels as though his own thoughts and opinions have not enough validity without people patting him on the back and agreeing.
There there, it's alright, you're not aloooone

But why do you want to hear that?
So that you can tell them that they're wrong or likewise?

My take:
I'm gonna go with "she created that character and story in her mind"...all the scenes featuring the guy, the truck included, was her creating a "villain" so as to further seperate her two ?personalities?. So while some scenes featured "her as the bad guy as her etc", some were just playing out in her head to further reinforce the personality to herself. The truck didn't exist. The female headless victim didn't exist. The crash was just her crashing, perhaps her other personality PUSHING to put them in a situation where she could dispose of her friend. Notice how though, she only created situations and events for this "bad guy"? No back story, no name, no reason for being? This pretty much fits in with it if you ask me...there was no history, as it was just her all along, no reasons, as she was simply fucked in the head. I'm not sure what other plot holes there were...have not seen it in some time.
