The best under-rated/ little known horror book?

Like the title says, I am looking for horror books that are great but don't have so much acclaim. Obviously no books by Stephen king or Clive Barker. What's the best horror book that you have read which is relatively unknown, and what's so good about it?

Garrett Cook's Murderland series, now collected in one volume, available on Amazon. Fierce, violent, subversive, hallucinatory. Involves a not too distant future where America worships serial killers, and follows their exploits on TV.
