Horror novel about patients taking over asylum
 
I came across this a year or two ago and completely spaced. Maybe a Dean Koontz novel? Or a Stephen King short story? Mental patients take over the institution and control it for a period of time. Any of this ring a bell?

Not quite. The story I'm thinking of had the mental patients take over the institution, set up a series of rules and their own society, and the outside world didn't know. No guards, no doctors, etc.
